COVID-19 Letter And Decision Tree

To patients of In2it Medical:

As the numbers of the COVID-19 virus continue to rise, we, at In2it Medical want to ensure that our patients remain protected and healthy. As such, we want to raise awareness among our patients on how to protect themselves, as well as provide a clean and safe environment at In2it Medical.

In order to prevent the spread of infection and provide peace of mind for our patients and staff. As part of our effort, we have implemented the use of Tele-Medicine office visits for those who prefer to or need to remain at home and would still like the option of visiting with our medical providers. Our Tele-Medicine option is offered through our patient app called MyCarePlan. This app is available on Apple and Android devices and is a great communication tool for our patients. You can download the app through the App Store https://apps.apple.com/us/app/mycareplan/id1130960154 or Google Play https://play.google.com/store/apps/details?id=com.qure4u.careplan&hl=en_US.

However, we do know that Tele-Medicine visits will not be ideal for many patients, as it does not allow for a complete physical exam or ultrasound imaging. For patients that prefer to or need to come to our office, we ask that you extend courtesy and consideration to our other patients and our staff and ensure you are healthy at the time of your visit. We have created a Decision Tree to help you determine whether or not you should keep your scheduled appointments at the office.

As always, please contact us if you have any concerns or questions related to your care at In2it Medical. Office, (801) 610-7321 and email, info@in2itmedical.com

We truly value our patients and we appreciate the support you provide to us. We wish you the best and hope you remain happy and healthy.

Sincerely,

Dr. Craig Chappell and the In2it Medical Team
